Where is Martin Lewis' Ghost Deal published?

I recently found a set of cards in my drawer labeled, "Martin Lewis' Ghost Deal"; however, I can't recall where in my library it has been published.

I'd sure appreciate it if someone would please give me the reference.

I did find one related reference in print: "Ghost Deal Variation", The Commercial Magic of JC Wagner, pg 121-126 but the original printed reference continues to elude me.

I think--THINK--it's called "Magician's Poker" in his 2001 lecture notes.

I just found my cards for "Ghost Deal" and it is the same setup as described originally as "Magician's Poker."
He still sells the notes on CD.
