I just got an e-mail from fellow canuck Paul Alberstat reminding me that, in addition to Jay Sankey, David Harkey published a pen-through-bill trick using a secret fold in Simply Harkey (1991). Japans Ken Kuroki also devised an intriguing method to accomplish the effect, which can be found in Five Times Five Japan (Kaufman, 1992).

I've not seen December or January yet, but Bob Neale has a version called the Pyramid of Immortality in Folding Money Fooling (it's also in one of his lecture notes, I forget which). Also, there's Harlan's Now-U-Z-it from an early Minotaur.

I saw David Harkey do his pen-through-bill from Simply Harkey (1991) at a lecture in sweden at the Magic Theatre owned by Stefan Schutzer.
It looked very good, it was a very fine effect.
