If you have a copy of Michael Ammar's book (The Magic of Michael Ammar), He has several forms which can be quite helpful. One is a performance evaluation. I have modified it a bit, as I'm sure you will as well, but it's a nice guide. If you don't have the book, it has more than enough great stuff in it to warrant your purchase in my opinion. Finally, I can email you a copy of my version if you'd like to look at it.
