Based on the photo, it appears to be the Freddie Jelsma book on Fred Kaps.

Under Jelsma is how I have it listed in my library catalog. The books are in storage but I seem to think that Jelsma acted more in the role of editor and compiler than pure writer for this title.

I've also never considered this pamphlet to be rare by any stretch.

The publisher lists it as "compiled by Freddie Jelsma, edited by Stephen Minch". It is hard to see how a book still in print ($16.75) can be considered "rare"...
