December 2017 Issue

I seem to recall a general rule to determine if an issue is lost, but I can't find it. My December issue hasn't arrived. at what point is it considered lost?

It's considered lost if you haven't received it by the end of the second week of the dated month. At the moment it's probably sitting in a pile of 10,000 Christmas catalogues at your local post office.
